- Collection and Use of Personal information
The Society collects personal information during the course of its business interactions, for those applying to be members of the Society.
Data and information collected by the Society for the use in the PHSANZ Registry is de-identified data. All data also undergoes review by individual Hospital Ethics Committees prior to being provided to the Society.
- Storage of Personal Information
Personal information is gathered from persons applying for membership to the Society. This personal information is stored in the Society’s Dropbox and Mail Chip.
Information gathered or provided to the Society is stored on the PHSANZ Registry. The Registry is hosted on a secure server and is managed by a third-party provider.
- Use and Disclosure of Personal Information
Personal information collected by PHSANZ is used for the sole purpose of membership and applications.
Over the course of business interactions the Society may disclose personal information to third party business services, to government agencies/regulatory bodies, where necessary to lessen or prevent a serious threat to the life, health or safety of any individual, if the person to which the personal information relates has consented and where disclosure is required or permitted by law.
- Individual Rights
You may at any time request access to the personal information the Society holds about you, notify the Society that personal information held is incorrect or request to stop receiving communications from the Society.
- Complaints
If you have a complaint regarding the way the Society has handled your personal information you can contact the Society through the Contact Us function on the website.
An employee or agent of the Society will investigate your complaint and provide a response accordingly. If you remain unsatisfied with the way that the Society handled your complaint, you can raise your complaint to the Office of the Australian Information Commissioner.
